We ask you to bring your insurance cards and any medications you are taking; also, bring any medical records you may have. You will need to complete a new patient registration form. This will require you to arrive 15 minutes before your appointment time. This information will assist us in helping you proceed smoothly through your examination.

Please call us at least 24 hours in advance if you cannot keep your appointment so we may give your time to another patient in need of our care.
We participate with and accept most types of health insurances. Our staff will gladly tell you if we participate in your particular health plan. We also accept cash and check. Our policy is that you pay any amount not covered by your insurance at the time of your visit.
When calling our office, our receptionist will bring your questions to the attention of your physician or the nurse as soon as possible. Please give us till the end of the business day to return your calls.
When calling to renew your prescription, please call at least 24 hours before you are out of your medication. Please have the name and dose of your medication as well as the name and phone number of your pharmacy available for our staff.
Form or Copy Fee:
There is a charge of $30.00 for all disability papers. In order to reduce the cost of copying medical records in our office we have implemented a copying charge. Medical records that need to be copied and sent out will be charge a $0.80 per page fee.
If you begin experiencing the symptoms of a stroke (numbness in an extremity, slurred speech, severe sudden headache, blurred or double vision or dizziness), recurrent seizures or seizures lasting more than five minutes, please go directly to the emergency room of the nearest hospital and have the attendant call our office immediately upon your arrival to allow notification of our on call neurologist.
